Vardenafil undergoes metabolism predominantly in the liver through the action of cytochrome P450 isoenzymes, primarily CYP3A4, along with CYP3A5 and various CYP2C isoforms. Additionally, the principal metabolite generated from the desethylation of vardenafil retains some degree of activity.
Vardenafil is primarily eliminated as metabolites through feces (91 to 95%), with a smaller proportion being excreted in the urine.
Vardenafil, a recently introduced PDE5 inhibitor for the oral treatment of male erectile dysfunction, exhibits considerable structural similarity to sildenafil (Viagra?), the first PDE5 inhibitor launched in 1998 for this purpose. The synthesis of vardenafil occurs through a three-step process, beginning with a cyclization reaction involving 2-ethyoxybenzamidine, 2-butyramidopropionic acid, and ethoxyallyl chloride to form the imidazotriazine ring system. This is followed by sulfonation to produce the corresponding sulfonyl chloride, which then undergoes condensation with 1-ethylpiperazine. Vardenafil demonstrates a potency for PDE5 inhibition (IC50=0.7 nM) that is approximately ten times greater than that of sildenafil (IC50=6.6 nM). Typically, vardenafil is administered in single doses of 10 mg or 20 mg. The time required to achieve maximum plasma concentration is 0.75 hours, which is marginally shorter than that of sildenafil (tmax=1.16 h) and tadalafil (tmax=2 h), with a half-life ranging from 4 to 5 hours. Although nearly completely absorbed upon oral administration, the mean absolute bioavailability of a 10 mg dose is about 15%, attributable to significant first-pass metabolism. Vardenafil is primarily metabolized in the liver by CYP3A4 and is predominantly excreted in feces. In clinical trials, vardenafil administered in doses of 10–20 mg was found to be both well tolerated and effective in patients experiencing erectile dysfunction (ED) of varying severities, including those with comorbid conditions such as diabetes mellitus, hypertension, or hyperlipidemia. The side-effect profile associated with vardenafil is comparable to that of sildenafil, with the most frequently reported adverse events being headache, flushing, dyspepsia, and nasal congestion. Vardenafil exhibits systemic vasodilatory effects, which may lead to a temporary reduction in supine blood pressure; however, this does not seem to result in any significant clinical implications. When compared to placebo, the average maximum reductions in supine systolic blood pressure following the administration of 20 mg and 40 mg of vardenafil were 6.9 mmHg and 4.3 mmHg, respectively. Furthermore, single and multiple oral doses of vardenafil, even at levels up to 40 mg, did not induce any clinically significant alterations in the ECGs of healthy male volunteers.
